VACCINE APPOINTMENT INFORMATION
Illinois and Suburban Cook County are currently distributing vaccines to eligible individuals in phases 1A and 1B (Health Care workers, senior citizens, those with underlying medical conditions, as well as some groups of frontline essential workers). The City of Chicago is currently prioritizing phase 1A (health care workers and senior citizens) and will move into phase 1b to accommodate more demographics with the rest of the state as vaccines become more available.
Those who qualify for a vaccine in either phase 1a or 1b can begin registering for appointments to get a vaccine using the following resources. Please keep in mind, vaccine availability is extremely limited. If no appointments are available on the sites below, make sure to register to receive notifications of openings when available to avoid spending hours on the phone or frequenting appointment websites.

For both Evanston & Skokie distributed vaccines, residents must pre-register using the respective links provided and will be notified when an appointment is available. These jurisdictions are following similar protocols, prioritizing elder residents and teachers.
For Evanston home-bound residents 65 years or older: please complete this form so the City can coordinate an in-home vaccination. This form can be filled out on someone’s behalf.

To Make Vaccine Appointments in Cook County by Phone: 833-308-1988

Please assist senior citizens/peers/loved ones who do not have access to the internet by sharing the Cook County Vaccine Hotline phone number above or assist them in making appointments. Cook County is currently the only jurisdiction with a hotline to make appointments.
UI Health is scheduling vaccination appointments for all Chicagoans age 65+ and frontline essential workers, including first responders, pre-K–12 education workers, childcare workers, grocery store employees, and postal service workers. To schedule, go to this website or call 833.53.MYVAX (833-536-9829) Monday through Friday 8 am to 5 pm.
Erie Family Health has COVID-19 vaccine appointments available for current Erie patients who are age 65 and older, healthcare workers, or frontline essential workers. If you meet these requirements, call 312-666-3494, Option 5, to schedule an appointment.

COVID-19 TESTING INFORMATION
While we wait for vaccines to become more widely available, it is still important to quarantine and get tested if you are experiencing any COVID like symptoms. Wearing masks and washing our hands are still some of the best prevention methods to protect ourselves and others in the fight against this pandemic. Doing so can also help reduce COVID-19 mutations to the vaccine. Testing information is available here .
MENTAL HEALTH RESOURCES
Call4Calm is free for anyone in Illinois experiencing stress and mental health issues related to COVID-19. Text “TALK” to 55020 and a mental health counselor will follow up within 24 hours.
SCAM WARNING
If you receive a call, text, or email saying that you can pay to receive early access to a COVID-19 vaccine or can pay to have a vaccine shipped to your home, DO NOT give your information to these individuals. The COVID-19 vaccine is free. Many scammers claim they are from the World Health Organization, the CDC, or a government official’s office.
